On behalf of the Office of Population Health, I am excited to share our inaugural Population Health Annual Report, which provides an update on the progress we have made towards our five-year strategic plan.
The Population Health five-year Strategic Plan was developed and launched in early 2024 to ensure that each of the office's five programs worked toward achieving goals aligned with our population health principles:
- Excellent primary care, close to home
- All children live healthier, with more effective and affordable healthcare
- Improved health and outcomes for children with physical and behavioral health needs
- Close equity gaps in care
- Be the leader in pediatric population health research and innovation
During 2024, the Office of Population Health made strides towards meeting those goals. Focusing particularly on our organization's Care and Community aspirations, the programs associated with Population Health align with Cincinnati Children's mission to be the leader in improving child health.
Highlights include:
- Increased primary care access through growth of the Community Health Services Network (CHSN)
- Tripled HealthVine monthly enrollment rate for care management
- Served 34% of the pediatric population within the service area with integrated behavioral health
- Elevated participation in and alignment of learning networks
- Expanded school-based health care delivery options through new services and expansion
This report details the accomplishments and collaborations of the programs as we move towards achieving the best health outcomes for every patient in our population.
We are incredibly grateful for the dedication and partnership of those who contribute to this work. In 2025, we are excited to build on this momentum and continue striving for the best health outcomes for every child -- locally, across our state, and beyond.
